Appointment Information
Making an appointment for an office visit is easy. You can phone our office or contact us by email to request an appointment. When sending an email, be sure to include your name, daytime phone number, and the best time to reach you. One of our office staff will return your inquiry promptly.
Prior to calling for an appointment, we recommend you check with your insurance company. Many insurance carriers require a referral from a primary care physician prior to making appointments with medical specialists. A list of the insurance carriers associated with Swan Surgical can be found in the insurance carrier section of our website.
If you have an appointment scheduled for your first visit to our office, please print out and complete the new patient forms found in the patient forms sections of our website. You can bring the completed forms in with you on your first visit.
